Salisbury GP Surgery

Important information for patients currently registered at Salisbury Surgery, SW6


We wrote recently to the registered patients of Salisbury Surgery, Dr R N Muthiah, 178 Dawes Road, Fulham, London, SW6 7HS to inform them that Dr Muthiah wishes to hand back the practice contract he holds with NHS England. This means Salisbury Surgery practice will close on 30 January 2021. The current premises will not be available for the provision of GP services after that date.

Share your views

The Primary Care Commissioning Committee of Hammersmith and Fulham CCG will be asked to make a decision on the future of the practice at its meeting in December. The CCG is likely to recommend that the practice is closed, asking patients to register elsewhere. However, before this recommendation is put to the Committee, the CCG wants to take into consideration any feedback from patients. It has set up a survey which you can access online. The deadline for responses is 2pm on Friday 18 December and this survey is for patients registered at Salisbury Surgery only.

Online drop-in sessions

The CCG will hold two online drop-in sessions for patients who wish to put across their comments in person. These will be held via Microsoft Teams on Wednesday 2 December at 10.00 and Tuesday 8 December at 15.00. If you would like to join one of these sessions please send an email to by 30 November and we will forward the joining instructions for the meeting.

1. If I want to register at another practice, do I need to do this myself?

Yes you do. Visit  for a list of practices near you. Please refer to the list below of GP practices within one mile of The Salisbury Surgery’s postcode. You should visit the GP practice website and register online through the online registration form. If you are unable to register this way, please contact the surgery with which you would like to register directly and ask them to register you. The practice will need to check that you live within their catchment area.

2. What if a practice refuses to register me?

If you live within the catchment area of a practice and they have an open list then they are contractually obliged to register you. All practices within a mile of The Salisbury Surgery have open lists.

3. Do I have to register with one of the practices on the list included with this letter?

No – you can register with any practice if you are in their catchment area[1]. You can find a surgery near you using the NHS Website which gives further information about practices, like opening hours and patient satisfaction survey results.

4. Will there be any changes to the services offered to me at another practice?

All GP practices are required to provide essential services to patients. However, it is possible that there may be some services offered by local GP surgeries which best meet your needs or those of your family members. Patients are advised to review  to decide which practice best meets their needs.

5. What happens to my medical notes?

If you register with another practice before 30 January 2021, your notes will be transferred securely to the new practice. If you do not register with another practice by then, your notes will be removed from the surgery and held securely by NHS England’s Primary Care Support Team. Later, when you do register, the new practice will ask the team to send your notes to them.
