Delivering better care out of hospital

Community Service Independence plus

Community Service Independence plus brings health and social care to your own home for up to six weeks.

This flagship health and wellbeing service offered by Hammersmith and Fulham CCG and local health and social care organisations has seen hundreds of patients supported and cared for through the service.

The service brings health and social care together and provides a team of experts working together to create a care plan. This includes your GP, a social worker, hospital consultant, community matron, nurses and therapists, a health and social care coordinator and your own case manager. 

Often people don’t know what to do if they’re in a crisis and naturally they call an ambulance or contact their GP. This service can help them so they don’t need to go to hospital. 

They only go to hospital if they really need to and if a patient has just come out of hospital the service helps them settle back into living at home.   

The service is offered free of charge and while patients are receiving care and support they can continue to see their GP as normal, as they remain responsible for their care.  If they need on going care after six weeks, the team will work with the patient and their GP to put the care in place. 

Community Independence Service plus is a partnership between Hammersmith and Fulham CCG, London Borough of Hammersmith and Fulham, Imperial College Health Care NHS Trust, Chelsea and Westminster Hospital NHS Foundation Trust and Central London Community Healthcare NHS Trust.
