Brent, Central London, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, and West London Clinical Commissioning Groups is transferring to the new CCG – North West London Clinical Commissioning Group on 1 April 2021. The new Clinical Commissioning Group will become the new data controller.

Views needed on community respiratory services in Hammersmith and Fulham

Hammersmith and Fulham Clinical Commissioning Group (CCG) are looking at the possibility of starting a combined cardiology and respiratory service in the community. 

What will the service do?

This service would assess and treat people with heart and/or lung conditions. These health issues can be very closely linked and people may suffer with symptoms such as breathlessness which could be caused by either cardiology or respiratory problems. 

Our local GPs and hospital doctors have proposed we start a new community service which provides integrated cardiology and respiratory care in the community at various locations in Hammersmith and Fulham.

The ideal way we would like to run the service includes:

  • Patients  having a say in decisions about their care
  • Joined up working between all healthcare staff
  • Finding the problem early and working with you to manage your own condition
  • Supporting you in all ways throughout your care

There is currently a Community Respiratory service which the CCG would evaluate and re-design as part of this process. 

We would like to hear your views. Please give us your feedback on the current Community Respiratory Service and the newly proposed combined Cardio-Respiratory Community Service.
