Hammersmith and Fulham CCG has commissioned Dulwich Medical Centre Healthcare Limited (DMC) to provide a new Community Dermatology service across the borough of Hammersmith and Fulham. This new service starts on the 6 November 2017, and will begin accepting referrals from 1 October. The service will replace the previous community dermatology service run by Central London Community Healthcare NHS Trust (CLCH).
The new service provides consultant-led community clinics to patients who are registered with a Hammersmith and Fulham GP. The service will deliver a high standard of consultant-led care for patients. The service will also offer effective, high quality, accessible care closer to home, with the aim of improving improve access through a single point of referral.
The new service will seek to provide patients with all of their treatment in one visit, where clinically appropriate. It will also have a strong emphasis on empowering and educating patients in effectively self-managing their dermatological conditions.
The service will include a pilot of tele-dermatology. Teledermatology involves securely sending images (with patient consent) to a Consultant Dermatologist to support accurate diagnosis.
